PreparedHealth: Jefferson, Prepared Health form digital health partnership to track post-acute care

Jefferson Health has selected Prepared Health to be its digital technology partner for post-acute and transitional care. Chicago-based Prepared Health has developed a technology platform that allows hospitals to connect, collaborate and react in real-time with the providers handling a patient's care after the patient is discharged.
