New FeaturesLace Index ScreeningWe're happy to announce the newest assessment in RoundingWell: The LACE Index Screening. The LACE Index identifies patients that are at risk for readmission or death within thirty days of hospital discharge. Embedding the screening tool in RoundingWell will allow clinicians to capture the assessment electronically. Scoring involves the following four parameters: (L)ength of stay of the index admission, (A)cuity of the admission, (C)o-morbidities, and (E)mergency Department visits within the last 6 months.Here are the LACE Index Screening scoring rules:Score: 0-4 = low risk for readmission. Low risk scores do not trigger the "LACE Index Screening" pathway, but will display on a custom patient list. Score: 5-9 = moderate risk for readmission. Moderate risk scores will result in the "LACE Index Screening" pathway with the factor: "Patient screening indicates moderate risk for readmission or death within 30 days. (LACE)" Score: >=10 high risk for readmission. High risk scores will result in the "LACE Index Screening" pathway with the factor "Patient screening indicates high risk for readmission or death within 30 days. (LACE)" Note: The LACE Index Screening has already been added to organizations that have submitted a request. This screening is not proprietary and does not require a private license to utilize, so it is open to all RoundingWell customers. Let your Account Manager know if you would like to have this new assessment turned on in RoundingWell.ENHANCEMENTSPatient SidebarWe have redesigned the Patient Sidebar with a few cosmetic updates. We're moving the links to Overview, Profile, and History button higher up on the Patient Sidebar in the desktop experience.Pathway Task SidebarToday is the first phase of an evolution to RoundingWell tasks. In the Pathway Builder, instead of creating or editing tasks in-line; you'll now see a new pane open up on the side of the page to create, edit, or delete task. Also, you will no longer have access to the quick task selections while creating a new task in the Pathway Builder. This lays the groundwork for future task elements. Bug Fixes We fixed a number of bugs and incidentals in an effort to help provide a better experience for you in the RoundingWell clinician console.
